If you are interested in hospice services, call us at 256-891-7724 and speak to our admissions specialist. - Complete a referral
The admissions specialist will get your information, contact your healthcare provider, and obtain medical records. These will help our healthcare professionals determine eligibility. An in-person assessment may be needed to help determine eligibility. - Initial Visit
During this visit, the SCH admissions team works with the patient and/or caregiver to complete an assessment, order medications and supplies, and begin a plan of care that best meets the patient’s goals.
